Complete Your Intake Forms
In order to start the process of becoming a new patient, your orthodontist will require you to fill out intake forms. This information will provide the orthodontic team with the information they need to provide you with the best care possible, including your insurance, health history, and emergency contacts.
If you’re a new patient at LakePointe Orthodontics, you can fill out your intake paperwork online.
In addition to this paperwork, be sure to bring the following to your appointment:
- Any additional X-rays from your dentist that are necessary for your pending dental work
